Job summary

Current PA license in good standing required; SNF/long-term care experience preferred; familiarity with Net Health EHR is a plus. On-site role focused on compassionate, quality care for residents.

Qualifications

• Licensed Physical Therapist in PA in good standing is required.
• Experience in SNF/long‑term care preferred.
• Familiarity with Net Health EHR is preferred.

Responsibilities

• Evaluate, plan, and administer treatment to residents to relieve pain and restore function.
• Deliver treatment per established care plans and modify as needed.
• Provide clinical support and educate patients, families, and caregivers.
• Measure and record motor function, strength, and muscle performance.
• Monitor patient responses and adjust treatments accordingly.
• Complete documentation per company policy and state/federal regulations.

The Physical Therapist is expected to follow facility core values in administering department services and helping to fulfill all therapy-related resident care needs.

Duties/Responsibilities:

• Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
• Assured all treatment is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
• Measures & records patient’s motor function, strength, and muscle performance
• Monitors patient responses to treatments and makes modifications during treatment sessions as necessary
• Completes documentation in accordance with company policy and state/federal regulations

Skills:

• SNF/Long term care experience preferred
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred
• Ability to manage patients with different types of personalities

Required licensure:

• Current License as a Physical Therapist in the State of PA. Must be in good standing.
