Job summary

Kadima Rehabilitation & Nursing at Latrobe SNF/LTC seeks a dedicated Physical Therapist to join our in-house rehab team. You will evaluate residents, plan treatments, and promote functional recovery in a skilled nursing setting.

You will document progress, collaborate with caregivers, and ensure care aligns with established plans. A PA license in good standing is required to practice in Pennsylvania; experience in SNF/long-term care is preferred.

Qualifications

• Evaluate and treat residents to relieve pain and restore function, following plan of care.
• Document outcomes and adjust treatment as needed within established plan.
• Must hold PA Physical Therapist license in good standing.

Responsibilities

• Evaluate, plan, and administer treatment to residents with disabilities to relieve pain and restore function.
• Ensure treatment is delivered according to an established plan of care.
• Provide clinical support and instruct residents, families, and caregivers.
• Measure and record motor function, strength, and muscle performance.
• Monitor patient responses to treatments and adjust sessions as necessary.
• Complete documentation per policy and state/federal regulations.

We are seeking a full-time Physical Therapist to join our amazing in-house rehab team at Kadima Rehabilitation & Nursing at Latrobe SNF/LTC

The Physical Therapist (PT) plays a pivotal role in the rehab department at our skilled nursing facility, promoting the physical well-being and functional recovery of our residents.

Duties/Responsibilities

• Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
• Assured all treatment is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
• Measures & records patient’s motor function, strength, and muscle performance
• Monitors patient responses to treatments and makes modifications during treatment sessions as necessary
• Completes documentation in accordance with company policy and state/federal regulations

Skills

• SNF/Long term care experience preferred
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred
• Ability to manage patients with different types of personalities

Required licensure

• Current License as a Physical Therapist in the State of PA. Must be in good standing.
