Job summary

American Senior Communities is seeking a Physical Therapist for PRN coverage at Trailpoint Village, Creekside Village, or Elkhart Meadows. You will implement skilled physical therapy services in ASC Therapies & Wellness settings, aligning with PT practice and center policies.

You will evaluate patients, develop treatment plans, supervise PTAs, contribute to case management, and ensure compliance with Indiana state regulations.

The Physical Therapist implements skilled physical therapy services for patients in all ASC Therapies & Wellness settings, in accordance with the principles and practices of physical therapy and within the policies and procedures.

• Evaluate patients and provides quality patient care
• Develop initial and ongoing treatment plans
• Supervise Physical Therapy Assistants (PTA)
• Contribute to case management
• Maintain positive level of interaction with patients and center staff
• Adhere to applicable state regulations concerning physical therapy services
