Job summary

American Senior Communities is seeking a PRN Physical Therapist Assistant (PTA) to provide skilled therapy under the supervision of a licensed physical therapist. The role covers South Bend, Mishawaka, and Elkhart with services across ASC Therapies & Wellness settings including home health, SNF, and outpatient care.

You will deliver direct therapy per the plan of care, monitor patient progress, and support discharge planning while maintaining positive interactions with patients and staff,

PRN Physical Therapist Assistant (PTA) Opportunity at Trailpoint Village

Availability for South Bend, Mishawaka, and Elkhart

The Physical Therapy Assistant assists in providing skilled care under the supervision of the registered physical therapist. Able to provide services for patients in all ASC Therapies & Wellness Settings. Able to provide services for patients in all ASC Therapies & Wellness Settings including home health, skilled nursing, outpatient therapies.

• Provide direct therapy to patients outlined in the OT's plan of care

• Collaborate with members for discharge plans
• Contribute to case management
• Maintain positive level of interaction with patients and center staff
• Adhere to applicable state regulations concerning occupational therapy
