Job summary

American Senior Communities is seeking a PRN Physical Therapist Assistant (PTA) to assist in providing skilled therapy under the supervision of a registered physical therapist across ASC Therapies & Wellness settings including home health, SNF and outpatient services in the South Bend area.

The PTA will deliver direct therapy per the plan of care, monitor patient progress, assist with discharge planning, and collaborate with the therapy team while upholding state regulations and the CARE values

Availability for South Bend, Mishawaka, and Elkhart

The Physical Therapy Assistant assists in providing skilled care under the supervision of the registered physical therapist. Able to provide services for patients in all ASC Therapies & Wellness Settings. Able to provide services for patients in all ASC Therapies & Wellness Settings including home health, skilled nursing, outpatient therapies.
