Job summary

American Senior Communities is seeking a PRN Physical Therapist Assistant (PTA) for Trailpoint Village serving South Bend, Mishawaka, and Elkhart. The PTA assists under the supervision of a licensed physical therapist and provides direct therapy across ASC Therapies & Wellness settings, including home health, SNF, and outpatient care.

The role emphasizes collaboration with licensed therapists, discharge planning, case management, and ensuring patient-centered care within state regulations.

PRN Physical Therapist Assistant (PTA) Opportunity at Trailpoint Village
Availability for South Bend, Mishawaka, and Elkhart

The Physical Therapy Assistant assists in providing skilled care under the supervision of the registered physical therapist. Able to provide services for patients in all ASC Therapies & Wellness Settings. Able to provide services for patients in all ASC Therapies & Wellness Settings including home health, skilled nursing, outpatient therapies.

• Provide direct therapy to patients outlined in the OT's plan of care


• Monitor and document patient progress


• Collaborate with members for discharge plans

• Maintain positive level of interaction with patients and center staff


• Adhere to applicable state regulations concerning occupational therapy
