Job summary

Senior Care Partners PACE is seeking a Clinical Care Registered Nurse (PRN) to provide direct nursing care to our program participants. The role works independently on advanced nursing tasks, collaborating with licensed and non-licensed staff within a PACE setting.

Responsibilities include triaging emergent cases, monitoring conditions, administering treatments, and performing home visits as assigned by the PCP. Requires Michigan RN license, CPR, and willingness to travel between centers.

Qualifications

• RN licensure (state) and CPR certification as required.
• Experience with caring for frail elderly or long-term care population.
• Willingness to travel between Senior Care Partners P.A.C.E. centers; valid Michigan driver’s license.

Responsibilities

• Provide direct nursing care to participants.
• Monitor acute and chronic medical conditions with PCP coordination.
• Administer medications and immunizations as prescribed.
• Perform assessments and participate in care plans.
• Conduct home visits and clinic care per PCP orders.
• On-call needs and after-hours coverage as needed.
• Document in medical records and maintain confidentiality.
