Job summary

Creative Solutions in Healthcare, Inc. is seeking a compassionate Occupational Therapy Assistant (OTA) to join our in-house rehab team on a PRN basis.

You will provide person-centered care to help residents achieve independence and improve quality of life in a Skilled Nursing Facility setting. The ideal candidate holds an active Texas OTA license and COTA credential, with strong communication and teamwork skills.

Qualifications

• Active Texas OTA license and COTA credential required.
• SNF or long-term care experience preferred.
• Knowledge of Net Health Documentation System preferred.

Responsibilities

• Plan and administer occupational therapy treatments to residents with disabilities.
• Provide skilled OT services under supervision of an OT.
• Respond to service requests and relay information to the OT.
• Ensure treatments align with the established plan of care.
• Provide clinical support and instruction to patients and caregivers.
• Monitor patient responses and report changes.
• Complete required documentation accurately and timely.
• Attend required Rehab Director meetings.
