Job summary

Creative Solutions in Healthcare, Inc. seeks a Per Diem Occupational Therapy Assistant (OTA) to join our in-house rehab team on a PRN basis in Texas.

You will deliver person-centered OT services under the supervision of an OT and contribute to improving residents’ independence and quality of life. The role requires an active Texas OTA license and COTA credential, with SNF or long-term care experience preferred.

Qualifications

• Active and valid Texas Occupational Therapy Assistant license.
• Certified Occupational Therapy Assistant (COTA) credential required.
• In good standing with regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Knowledge of occupational therapy treatment practices.
• Experience with Net Health EHR preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication and teamwork skills.

Responsibilities

• Plan and administer occupational therapy treatments to residents and patients to relieve pain and improve function.
• Provide skilled OT services under supervision of an OT.
• Relay information and referrals to the OT for service requests.
• Deliver treatments according to the established plan of care.
• Provide clinical support and instruction to patients, families, and caregivers.
• Monitor patient responses and report changes as appropriate.
• Complete required documentation in a timely manner per policy and regulations.
• Attend required Rehab Director meetings.
