Job summary

Creative Solutions in Healthcare, Inc. is seeking a Per Diem (PRN) Occupational Therapy Assistant (OTA) to join our in-house rehab team. You will deliver person-centered OT services to residents, under supervision, on a PRN basis.

Responsibilities include planning and administering treatments, documenting outcomes, and collaborating with the OT and care teams to promote independence and quality of life.

Qualifications

• Active and valid Texas OTA license is required.
• Certified Occupational Therapy Assistant (COTA) credential required.
• SNF or long-term care experience is preferred.
• Knowledge of Net Health Electronic Documentation System is a plus.
• Strong communication and teamwork skills are essential.

Responsibilities

• Plan and administer occupational therapy treatments to residents to relieve pain, restore or improve function.
• Provide skilled OT services under supervision of an Occupational Therapist according to physician orders.
• Respond to service requests by relaying information to the Occupational Therapist.
• Ensure all treatments follow the established plan of care and documentation is complete and timely.
• Provide clinical support and instruction to patients, families, and caregivers.
• Monitor patient responses and report changes as appropriate.
• Complete required documentation per company policy and regulations.
• Attend required meetings as designated by the Director of Rehab.
