Job summary

Creative Solutions in Healthcare, Inc. in Texas is seeking a Per Diem Occupational Therapy Assistant (OTA) to join our in-house rehab team. This PRN role provides person-centered OT services to residents in our skilled nursing facility.

The OTA will plan and administer treatments under an Occupational Therapist supervision, maintain documentation, and participate in care planning meetings. Texas license and COTA credential required; SNF experience preferred; strong communication and teamwork

Qualifications

• Active and valid Texas OTA license required.
• Certified Occupational Therapy Assistant (COTA) credential required.
• In good standing with regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Capable of managing patients with varying personalities and needs; strong communication skills.

Responsibilities

• Plan and administer OT treatments to residents with temporary or long-term disabilities to relieve pain, restore or improve function.
• Provide skilled OT services under supervision of an OT.
• Respond to service requests by relaying information and referrals to the OT.
• Ensure treatments align with the established plan of care.
• Provide clinical support and instruction to patients, families, and caregivers.
• Monitor patient response and report changes as appropriate.
• Complete required documentation thoroughly and timely.
• Attend required meetings with Rehab leadership.
