Job summary

Creative Solutions in Healthcare, Inc. is seeking a Per Diem Occupational Therapy Assistant (OTA) to join our in-house rehab team. You will plan and deliver person-centered OT services under supervision, ensuring care plans are followed and documentation is timely.

This PRN role supports residents in a skilled nursing setting. Ideal candidates hold a Texas OTA license and COTA credential; SNF or long-term care experience is preferred.

Qualifications

• Texas OTA license (active and in good standing)
• COTA credential required
• SNF/long-term care experience preferred
• Knowledge of OT treatment practices
• Experience with Net Health EDS

Responsibilities

• Plan and administer occupational therapy treatments per plan of care
• Provide skilled OT services under supervision of an OT
• Document thoroughly and on time per policy and regulations
• Attend required Rehab Director meetings
