Job summary

Creative Solutions in Healthcare, Inc. is seeking a Per Diem Occupational Therapy Assistant (OTA) to join our in-house rehab team. The PRN role focuses on person-centered care to improve residents’ independence and quality of life.

You will plan and deliver OT services under supervision of an OT, maintain documentation, and collaborate with families and caregivers. Texas licensure and COTA credential are required; SNF experience is preferred.

Qualifications

• Active and valid Texas OT license.
• Certified Occupational Therapy Assistant (COTA) credential required.
• In good standing with regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication and teamwork skills.

Responsibilities

• Plan and administer OT treatments to residents to relieve pain and improve function.
• Provide OT services under supervision of an Occupational Therapist.
• Respond to service requests and relay information to the OT.
• Ensure treatments align with the established plan of care.
• Provide clinical support and instruction to patients, families, and caregivers.
• Monitor patient response to treatment interventions and report changes.
• Complete required documentation thoroughly and timely in accordance with policy and regulations.
• Attend required meetings as designated by the Director of Rehab.
