Job summary

Creative Solutions in Healthcare, Inc. is seeking a compassionate Per Diem (PRN) Occupational Therapy Assistant (OTA) to join our in-house rehab team.

You will provide person-centered care to help residents achieve independence and better quality of life. As an OTA on a PRN basis, you will plan and administer treatments, work under supervision of an OT, and collaborate closely with the rehab staff to meet individualized goals.

Qualifications

• Active and valid Texas Occupational Therapy Assistant license.
• Certified Occupational Therapy Assistant (COTA) credential required.
• In good standing with regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Current knowledge of OT treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication and teamwork skills.

Responsibilities

• Plan and administer occupational therapy treatments to residents to relieve pain and improve function.
• Provide skilled OT services under supervision of an Occupational Therapist.
• Respond to service requests by relaying information to the OT.
• Ensure treatments align with the plan of care.
• Provide clinical support and instruction to patients, families, and caregivers.
• Monitor patient response and report changes as appropriate.
• Complete required documentation thoroughly and timely per policy and regs.
• Attend required meetings designated by the Director of Rehab.
