Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our team and evaluate and treat patients in SNF settings. You will follow individualized plans of care, collaborate with facility staff, and supervise Occupational Therapy Assistants to ensure high-quality patient outcomes.

You will re-evaluate progress, revise goals as needed, and document evaluations, progress notes, and discharges in a timely manner.

Qualifications

• Qualified Occupational Therapist licensed to practice.
• Experience evaluating and treating patients in SNF settings.
• Ability to document progress and outcomes clearly.

Responsibilities

• Evaluate and treat patients in a SNF setting following an individualized plan of care.
• Develop plans of care and set measurable, functional goals.
• Treat patients and educate them on injury, progress and outcomes.
• Re-evaluate progress and revise plans of care as needed.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervise Occupational Therapy Assistants.
• Collaborate with facility staff to ensure excellent patient care.
