Job summary

Enhance Rehabilitation in Illinois is seeking a qualified Occupational Therapist to join our SNF team. You will evaluate and treat patients following an individualized plan of care.

You will collaborate with facility staff to ensure high-quality patient care, set measurable goals, and educate patients and families on progress and outcomes. You will also document all evaluations and progress, re-evaluate as needed, and supervise Occupational Therapy Assistants to maintain excellence in service

Responsibilities

• Evaluate and treat patients in a SNF setting following an individualized plan of care.
• Provide excellent care for our patients and exceptional service to patients and families.
• Collaborate with facility staff to ensure high-quality patient care.
• Evaluate new patients, develop plans of care, and set measurable, functional goals.
• Treat patients, address concerns, and educate them on injury, progress and probable outcomes.
• Re-evaluate patient progress, revise plans of care, and reset goals.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervise Occupational Therapy Assistants.
