Job summary

Outcome HC is seeking an Occupational Therapist (OT) to join a skilled nursing facility on a PRN basis. The OT will evaluate residents, develop individualized plans, and deliver therapies to promote independence and safety in daily activities.

Responsibilities include direct therapy, ADL retraining, mobility training, cognitive interventions, documentation, and collaboration with families and the care team. Prior SNF or geriatric experience is preferred but not required.

Qualifications

• Current state OT license.
• Strong clinical reasoning and communication skills.
• Experience in a skilled nursing facility or geriatric setting preferred but not required.
• Ability to work independently and as part of a collaborative team.

Responsibilities

• Conduct comprehensive evaluations of residents’ physical, cognitive, and psycho-social abilities.
• Develop individualized treatment plans focused on functional improvement, ADLs, mobility, and safety.
• Provide direct therapy services including therapeutic exercises, ADL retraining, cognitive interventions, and adaptive equipment training.
• Recommend and train residents on the use of assistive devices and adaptive equipment.
• Document evaluations, progress notes, and discharge summaries in accordance with regulatory and facility standards.
• Communicate treatment goals and progress with residents, families, and caregivers.
• Provide education on safe mobility, fall prevention, and home safety strategies.
• Maintain accurate and timely documentation for billing and reimbursement.
• Adhere to facility policies, professional ethics, and best‑practice standards.
