Clarion Wellness in Clarion, IA is seeking a licensed Occupational Therapist to join our post-acute care team on a PRN basis. You will evaluate and treat geriatric patients in long-term and short-term rehab settings, and develop individualized plans of care.

You will document progress, discharge summaries, and collaborate with a multidisciplinary team to optimize functional outcomes, while upholding CAPLICO values and patient-centered care.

Qualifications

• Licensed Occupational Therapist eligible to practice in Iowa.
• Ability to evaluate and treat geriatric patients in post-acute settings.
• Capability to develop and document plans of care and discharge summaries.

Responsibilities

• Screen and evaluate patients with cognitive and functional disorders and develop plans of care.
• Provide rehabilitative treatment and use standardized assessments.
• Document progress, adjust treatment plans, and discharge patients per regulations.
• Communicate with patients, families, and the health team regarding progress and plans.
