Job summary

Outcome HC is seeking a dedicated Occupational Therapist (OT) to join our interdisciplinary team in a skilled nursing facility. The OT will evaluate residents, develop individualized treatment plans, and deliver therapeutic interventions to promote independence and safety.

Responsibilities include direct therapy, ADL retraining, cognitive interventions, and collaboration with families and caregivers, while maintaining compliant documentation and ethical standards.

Qualifications

• Current state OT license is required.
• Strong clinical reasoning and communication skills.
• Experience in a skilled nursing facility or geriatric setting is preferred but not required.
• Ability to work independently and as part of a collaborative team.

Responsibilities

• Conduct comprehensive evaluations of residents' physical, cognitive, and psycho-social abilities.
• Develop individualized treatment plans focusing on ADLs, mobility, and safety.
• Provide direct therapy services including therapeutic exercises, ADL retraining, cognitive interventions, and adaptive equipment training.
• Document evaluations, progress notes, and discharge summaries in accordance with regulatory and facility standards.
• Communicate treatment goals and progress with residents, families, and caregivers.
