Job summary

HealthMark Rehabilitation seeks a PRN Occupational Therapist/OTR to treat residents and short‑term patients at Harbor Lakes facility in Granbury, Texas. The role requires Texas OTR licensure, strong communication, and the ability to manage care plans within a multi-disciplinary team.

Responsibilities include providing therapy per orders, documenting progress, and maintaining a safe rehab area. Prior geriatric rehab experience is preferred and a background check will be completed.

Qualifications

• Texas license/certification for OT/OTR.
• Strong oral and written communication skills.
• Organizational and analytical skills required.
• Ability to interact effectively with staff at all levels.
• Time management and good documentation practice.

Responsibilities

• Provides therapy services per physician’s orders and accepted practices.
• Develops and follows the plan of care with treatment, goals, and frequency.
• Performs skilled treatments and documents progress and changes.
• Completes timely documentation and billing in accordance with regulations.
• Maintains rehab gym and treatment area with infection prevention practices.
