Job summary

HealthMark Rehabilitation in Granbury, TX is seeking a PRN Certified Occupational Therapy Assistant (COTA) to provide skilled therapy to residents and short-term patients at Harbor Lakes Nursing & Rehabilitation Center.

You will follow physician orders, develop a plan of care, document progress, and participate in discharge planning while adhering to state and federal regulations. Texas licensure as a COTA is required.

Qualifications

• Must hold Texas license/certification for COTA.
• Strong oral and written communication skills required.
• Ability to function effectively in multi-faceted systems and interact with all levels of staff.
• Demonstrates time management and organizational skills.
• Experience in geriatric rehabilitation setting preferred.

Responsibilities

• Provides therapy services in accordance with physician’s orders and accepted professional practices.
• Develops an overall therapy plan of care including treatment, goals, and frequency.
• Performs skilled treatments according to the plan of care.
• Documents observations and patient progress within required timeframes.
• Completes all documentation timely and in accordance with state and federal regulations.
• Performs billing for services rendered as required.
• Maintains rehab gym equipment and treatment area with infection prevention practices.
