Job summary

HealthMark Rehabilitation is seeking a PRN Physical Therapist to treat residents and short-term patients at Harbor Lakes facility in Granbury, Texas. The role requires licensure in Texas with strong communication, organization, and clinical judgment to support discharge planning and coordinated care.

The therapist will evaluate patients, develop and implement plans of care, document progress, and collaborate with facility staff to maintain quality rehabilitation services on-site at Harbor Lakes

Qualifications

• Must hold Texas license/certification for PT (as applicable).
• Experience in geriatric rehabilitation preferred.
• Successful completion of hiring process / background check.

Responsibilities

• Provides therapy services per physician orders and professional standards.
• Develops plan of care with short and long term goals to support discharge planning.
• Performs skilled treatments according to the plan of care.
• Documents observations and patient responses within required timeframes.
• Maintains timely documentation in accordance with regulations.
• Completes billing for services rendered per policy.
• Maintains rehab gym equipment and treatment area with infection prevention in mind.
