Job summary

HealthMark Rehabilitation, which operates Harbor Lakes Nursing & Rehabilitation Center in Granbury, Texas, is seeking a PRN Certified Occupational Therapy Assistant (COTA) to deliver skilled therapy to residents.

The ideal candidate holds a Texas COTA license, demonstrates strong communication and time-management skills, and thrives in geriatric rehabilitation. You will develop care plans, provide treatments, document progress, and help maintain a safe, clean rehab space.

Qualifications

• Must hold Texas license/certification for COTA.
• Experience in geriatric rehabilitation preferred.
• Strong oral and written communication skills.
• Ability to function in multi-disciplinary teams and manage time effectively.
• Ability to meet physical requirements and complete documentation timely.

Responsibilities

• Provides therapy services in accordance with physician orders and accepted professional practices.
• Develops an overall plan of care to include treatment and frequency, with measurable goals.
• Performs skilled treatments according to established plan of care.
• Documents observations, patient progress, and changes within required timeframes.
• Completes all required documentation and billing on the date of service.
• Maintains rehab gym equipment and treatment area with infection prevention in mind.
