Zusammenfassung

Oakmont Healthcare and Rehabilitation of Humble is seeking a compassionate PRN Occupational Therapist to evaluate residents, develop individualized treatment plans, and support independence in daily activities. You will collaborate with the in-house rehab team, ensure timely documentation, and adhere to regulations.

Texas OT license is required; SNF/long-term care experience is preferred. New graduates are welcome to apply and may enjoy flexible scheduling as part of the PRN arrangement.

Qualifikationen

• Active and valid Texas Occupational Therapy license.
• SNF or long-term care experience preferred.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration, and clinical decision-making skills.

Aufgaben

• Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life
• Assess and interpret patient evaluations and test results
• Develop individualized treatment plans using a variety of therapeutic techniques to maximize performance in activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
• Assess patients home and living environments and recommend modifications based on their needs and capabilities
• Recommend adaptive equipment and provide training to support independence and safety
• Ensure all treatments are delivered in accordance with the established plan of care
• Monitor patient response to treatment and modify interventions as clinically appropriate in collaboration with the interdisciplinary team
• Provide clinical education and support to patients, family members, and caregivers
• Collaborate with interdisciplinary team members to ensure high-quality patient care
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
• Attend required meetings as designated by the Director of Rehab
