Job summary

Enhance Rehabilitation in Des Plaines, IL is seeking a qualified Occupational Therapist to join our dynamic team. In this role you will evaluate and treat patients in a SNF setting following an individualized plan of care.

You will document progress, re-evaluate goals, and supervise Occupational Therapy Assistants to maintain high-quality care and outcomes for patients.

Qualifications

• Qualified Occupational Therapist with clinical experience
• Ability to work in a SNF setting
• Strong communication with patients and families

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
