Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our dynamic team in a SNF setting. You will evaluate and treat patients based on an individualized plan of care and work closely with facility staff to ensure high-quality patient outcomes.

You will re-evaluate progress, update plans of care, and document evaluations, re-evaluations, discharges, and daily notes while supervising Occupational Therapy Assistants to maintain care standards.

• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
