Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our team in a SNF setting. You will evaluate patients, develop plans of care, and set measurable functional goals.

You will supervise Occupational Therapy Assistants, complete timely documentation, and collaborate with facility staff to ensure excellent patient care and educate patients on progress and prognosis.

Qualifications

• Licensed or eligible Occupational Therapist with state credentials.
• Experience in SNF setting preferred.
• Strong communication with patients and families.

Responsibilities

• Evaluate and treat patients in a skilled nursing facility following a plan of care.
• Develop plans of care and set measurable functional goals.
• Provide patient education on injury, progress, and outcomes.
• Supervise Occupational Therapy Assistants and document progress.
• Collaborate with facility staff to ensure high-quality patient care.
• Complete timely documentation including evaluations, re-evaluations, and discharges.

As an Enhance Rehabilitation Occupational Therapist, you will be a part of growing team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care.

• Providing excellent care for our patients


• Providing excellent customer service to our patients and family members


• Collaborate with facility staff to ensure excellent care for you patients


• Evaluating new patients, developing plans of care and setting measurable, functional goals


• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes


• Re-evaluating patient progress, revising plans of care and resetting goals


• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.


• Supervising Occupational Therapy Assistants.
