Job summary

Eventide on Eighth Care Center is seeking a Licensed Practical Nurse (LPN) for PRN (As Needed) shifts. 12-hour Day (630a-7p) or NOC (6:30p-7:00a) options, with a minimum of 24 hours per month and two weekend shifts each month.

Active MN LPN license or ability to obtain upon hire; LTC experience preferred. Responsibilities include nursing assessments, medication administration, documentation, and coordinating resident care per policies.

Qualifications

• Active MN LPN license or ability to obtain upon hire.
• Long Term Care experience preferred.
• LPN assesses residents’ needs via physical assessments and interviews.
• Provides direct nursing care according to policies and standards.

Responsibilities

• Administers medications and treatments and monitors effectiveness.
• Provides direct nursing care to residents per established policies and standards.
• Documents accurately in residents’ electronic medical records.
• Coordinates resident care conferences and communicates with families.
• Reports resident needs and incidents to the Charge Nurse or physician.

Skills

Active MN LPN license
