Job summary

Ohio Veterans Homes in Sandusky, OH is recruiting a PRN Licensed Practical Nurse (LPN) to provide direct resident care on a flexible, intermittent basis. The LPN reports to the medical team, administers medications, observes patient conditions, and documents care using the Point-Click-Care system.

Qualifications

• Active LPN license in Ohio.
• Experience in direct resident care settings preferred.
• Ability to document care and communicate with the medical team.

Responsibilities

• Assist residents with medical treatments and medications.
• Observe and report residents’ conditions to the medical team.
• Maintain care plans, progress notes, and monthly summaries.
• Provide direct nursing care including ADLs, vital signs, mobility assistance.
• Attend team meetings and trainings; comply with infection control measures.
