Job summary

VitalCore Health Strategies is seeking a Licensed Practical Nurse (LPN) for PRN Day and Evening shifts at Old Colony Correctional Center in Bridgewater, MA. The role delivers quality care within the LPN scope, following state practice acts and facility policies.

Responsibilities include monitoring patients, administering medications, documenting in MAR, and supporting health promotion and restoration efforts. CPR certification and MA nursing licensure are required.

Qualifications

• Graduate from a Licensed Practical Nursing program.
• Currently licensed as a Practical Nurse in the state of employment.
• Possesses an active CPR certification.
• Remains knowledgeable about specific state laws and regulations governing practice.
• Satisfactory completion of initial and annual clinical competencies to demonstrate aptitude as assigned by role.

Responsibilities

• Utilizes a systematic approach to meet the health needs of each individual patient.
• Implements nursing care within the LPN's scope of practice and complies with applicable laws.
• Assists in development and implementation of teaching plans based on patient needs.
• Cares for wounds with cleaning and dressing/bandaging.
• Administers medications to offenders.
• Monitors vital signs and reports changes to medical staff.
• Documents actions in MAR and medical records.
• Other nursing duties as assigned by facility.
