Job summary

InVision Customized Services is seeking a skilled Licensed Practical Nurse to support care coordination for people with IDD in Reading, PA. This on-site role emphasizes clinical oversight, medical records, and collaborative patient advocacy.

You will collaborate with physicians, staff, and community providers, maintain HIPAA-compliant documentation, and participate in health-related goal setting and outcomes reporting.

Qualifications

• Licensed practical nurse with PA licensure.
• Experience with IDD population is a plus.
• Experience with Electronic Health Records (EHR).
• Excellent written and verbal communication skills.

Responsibilities

• Coordinate medical needs and treatment for people supported.
• Review medical history and ensure follow-up care.
• Serve as liaison between internal teams and providers.
• Provide staff with education on diagnoses and medication management.
• Develop protocols for conditions affecting IDD population.
• Attend team meetings and report health goals and outcomes.
• Administer medications and accompany to medical appointments as needed.
