Job summary

Qualifications

• Licensed practical nursing license (PA) required.

Responsibilities

• Review and maintain medical history and records as it relates to diagnoses, physical health, health prevention, medical appointments, and follow-up care.

• Provide staff education on diagnoses, medications, and health protocols.
• Travel within the service region for home visits and meetings as needed.
