Job summary

InVision Customized Services is seeking a nursing professional to coordinate medical care for individuals supported by the program. The role involves reviewing medical histories, coordinating with physicians, and acting as an intermediary between internal teams and community providers.

The ideal candidate holds a PA Licensed Practical Nurse credential, brings experience with complex medical needs, and can travel within the service region.

Qualifications

• Current PA Licensed Practical Nurse.
• Professional experience with IDD population is a plus.
• Experience with Electronic Health Records (EHR) is preferred.

Responsibilities

• Review and maintain medical history and records related to diagnoses, health, and follow-up care.
• Coordinate care with physicians and community providers per regulations and best practices.
• Serve as liaison between InVision and providers including primary care, specialists, and regulators.
• Educate staff and people supported on medical diagnoses, medications, and dietary guidelines.
• Develop standardized protocols for conditions affecting IDD populations, including seizures and sepsis.
• Observe and prompt medical interventions when needed.
• Participate in team meetings and health-related goal setting and reporting.
• Obtain vital signs and perform basic health assessments as needed.
• Accompany people and staff to medical appointments as required.
• Support adherence to HIPAA and regulatory requirements.
