Job summary

Advancing Opportunities is seeking a Licensed Practical Nurse (LPN) to provide direct care and coordinate health services for individuals with intellectual and developmental disabilities. The role involves nursing assessments, medication administration, documentation, and collaboration with physicians and the care team.

The LPN will maintain health records, educate staff, and support clinical planning across residential and community-based settings.

Qualifications

  • High School Diploma or GED required.
  • Graduate of an accredited LPN program.
  • Current and unrestricted LPN license required.
  • Minimum of 1 year nursing experience with IDD/behavioral health preferred.
  • Ability to complete all required background checks.

Responsibilities

  • Provide prescribed treatments and healthcare procedures as ordered by a physician.
  • Conduct nursing assessments and contribute to individualized healthcare plans.
  • Monitor health status and changes in medical conditions.
  • Administer and monitor medications per MARs and regulations.
  • Provide health education and medical training to staff and individuals served.
  • Coordinate referrals and follow-up care with external providers.
  • Maintain accurate documentation in electronic health records and ISP medical reports.
  • Train and support newly hired staff on medical needs and protocols.
