Apollo Medical Home Health - a LiveWell Partners Network Agency seeks a compassionate Home Health LPN to work one-on-one with patients in their homes, providing education and skilled nursing to promote independence. Qualifications

• 1 year of LPN experience in home health, med-surg, or ED.
• Current LPN license in the state.
• CPR certification and valid driver’s license.
• Ability to work a flexible schedule.
• Excellent interpersonal skills.

Responsibilities

• Participate in planning and coordination of total patient care with the RN and physician’s plan of care, following the nursing care plan for each patient.
• Accurately report and record the patient’s condition and care, including signs and symptoms indicative of change.
• Prepare clinical/progress notes and summaries.
• Assist the patient with activities of daily living while teaching appropriate self-care techniques.
• Follow universal precautions and all emergency procedures during patient care.

Company:

Apollo Medical Home Health - a LiveWell Partners Network Agency

Compensation:

Pay per visit ($38 - $40 per routine visit), Mileage reimbursed at $0.67/mile, Payroll processed bi-weekly (26 pay periods).

Shift Type:

PRN (as needed), Minimum availability for 6 visits per week, Flexibility, In the field.

Summary:

We are looking for a compassionate Home Health Licensed Practical Nurse (LPN) to work one-on-one with our patients in their home environment to give proper education and skilled nursing to allow our patients to become more independent. The Home Health Licensed Practical Nurse (LPN) for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.

Responsibilities:

• Current state license as a Licensed Practical Nurse (LPN).

• Ability to work and flexible schedule.
• Excellent interpersonal skills.
