We are seeking compassionate Licensed Practical Nurses (LPNs) to work one-on-one with patients in their homes, providing education and skilled nursing to promote independence. You will join a team of supportive clinicians and maintain discipline to ensure the highest standard of care.

Qualifications

• 1 year experience as an LPN in home health, med surg, or ED setting.

• CPR certification and valid Driver’s license.

Compensation:

Pay per visit ($38 - $40 per routine visit), $2,280 - $2,400+ bi-weekly, Per visit incentive pay in addition to the minimum of the 30 visit expectation per week, Mileage reimbursed at $0.67/mile, Payroll processed bi-weekly (26 pay periods).

Shift Type:

Full-time, Monday - Friday, 8 AM to 4 PM (flexible), In the field/On the road at 15-30% of the average 40 hour work week.

Summary:

The LPN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.

• Participate in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care, and follow the nursing care plan for each patient.

• Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change.

• Prepare clinical/progress notes and summaries.

• Assist the patient with the activities of daily living while teaching appropriate self-care techniques.

• Follow universal precautions and all emergency procedures whenever giving any aspect of patient care.

Qualifications:

• 1 year experience as an LPN in a home care, med surg, or ED setting.

• Current state license as an LPN.
