Job summary

LiveWell Partners is seeking compassionate Licensed Practical Nurses (LPNs) to provide one-on-one education and skilled nursing care in patients' homes, enabling greater independence. The LPN will work with a team of supportive clinicians while maintaining discipline to ensure the highest standard of care.

The role emphasizes home health delivery, collaboration with RNs and physicians, and adherence to care plans with flexible scheduling and a patient-centric approach.

Qualifications

• 1 year experience as an LPN in home care, med surg, or ED setting.
• Current state license as an LPN.
• Proof of CPR certification and valid Driver’s license.
• Ability to work a flexible schedule.
• Excellent interpersonal skills.

Responsibilities

• Participate in planning and coordination of total patient care with the RN and physician's plan of care, and follow the nursing care plan for each patient.
• Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change.
• Prepare clinical/progress notes and summaries.
• Assist the patient with activities of daily living while teaching appropriate self-care techniques.
• Follow universal precautions and all emergency procedures whenever giving any aspect of patient care.

Licensed Practical Nurse (LPN)

Company: Apollo Medical Home Health - a LiveWell Partners Network Agency

Compensation: Pay per visit ($38 - $40 per routine visit), $2,280 - $2,400+ bi-weekly, Per visit incentive pay in addition to the minimum of the 30 visit expectation per week, Mileage reimbursed at $0.67/mile, Payroll processed bi-weekly (26 pay periods).

Shift Type: Full-time, Monday - Friday, 8 AM to 4 PM (flexible), In the field/On the road at 15-30% of the average 40 hour work week.
