Job summary

Empower Home Health, part of the LiveWell Partners network, is seeking a compassionate Licensed Practical Nurse (LPN) to provide in-home skilled nursing and education to patients in the Huntley, IL area. This field role emphasizes one-on-one patient care with a supportive clinical team.

Qualifications

• 1 year experience as an LPN in a home care, med surg, or ED setting.
• Current state license as an LPN.
• Proof of CPR certification and valid Driver’s license.

Responsibilities

• Participate in planning and coordination of total patient care with RN and physician's plan of care.

Compensation

Pay per visit ($38 - $40 per routine visit), $2,280 - $2,400+ bi-weekly, Per visit incentive pay in addition to the minimum of the 30 visit expectation per week, Mileage reimbursed at $0.67/mile, Payroll processed bi-weekly (26 pay periods).

Shift Type

Full-time, Monday - Friday, 8 AM to 4 PM (flexible), In the field/On the road at 15-30% of the average 40 hour work week.

Summary

We are looking for compassionate Licensed Practical Nurses (LPNs) to work one-on-one with our patients in their home environment to give proper education and skilled nursing to allow our patients to become more independent. The LPN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.

Responsibilities

• Participate in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care, and follow the nursing care plan for each patient.

• Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change.

• Prepare clinical/progress notes and summaries.

• Assist the patient with the activities of daily living while teaching appropriate self-care techniques.

• Follow universal precautions and all emergency procedures whenever giving any aspect of patient care.

Qualifications

• 1 year experience as an LPN in a home care, med surg, or ED setting.

• Current state license as an LPN.

• Proof of current CPR certification and valid Driver’s license.

• Ability to work and flexible schedule.

• Excellent interpersonal skills.
