Job summary

Apollo Medical Home Health is seeking compassionate Licensed Practical Nurses (LPNs) to provide skilled nursing care in patients’ homes. You will work one-on-one with patients, educate families, and collaborate with a team of clinicians to ensure high-quality care.

Qualifications

• 1 year experience as an LPN in home care, med surg, or ED setting.
• Current state license as an LPN.
• Proof of CPR certification and valid Driver’s license.
• Ability to work and flexible schedule.

Responsibilities

• Participate in planning and coordination of total patient care with the RN and physician’s plan of care.
• Report and record patient condition and care, including signs of change.
• Prepare clinical/progress notes and summaries.
• Assist patient with activities of daily living and teach self-care techniques.
• Follow universal precautions and emergency procedures during patient care.

Licensed Practical Nurse (LPN)

Company: Apollo Medical Home Health - a LiveWell Partners Network Agency

Compensation: Pay per visit ($38 - $40 per routine visit), $2,280 - $2,400+ bi-weekly, Per visit incentive pay in addition to the minimum of the 30 visit expectation per week, Mileage reimbursed at $0.67/mile, Payroll processed bi-weekly (26 pay periods).

Shift Type: Full-time, Monday - Friday, 8 AM to 4 PM (flexible), In the field/On the road at 15-30% of the average 40 hour work week.

Summary:

We are looking for compassionate Licensed Practical Nurses (LPNs) to work one-on-one with our patients in their home environment to give proper education and skilled nursing to allow our patients to become more independent. The LPN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.

Responsibilities:

• Participate in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care, and follow the nursing care plan for each patient.
• Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change.
• Prepare clinical/progress notes and summaries.
• Assist the patient with the activities of daily living while teaching appropriate self-care techniques.
• Follow universal precautions and all emergency procedures whenever giving any aspect of patient care.
