Job summary

Apollo Medical Home Health - a LiveWell Partners Network Agency is seeking a compassionate Home Health LPN to provide one-on-one nursing in patients' homes. You will educate patients and deliver skilled nursing care with a collaborative team of clinicians.

The role requires a 6-visit-per-week minimum, PRN availability, and a focus on patient independence and safety. CPR certification, an LPN license, and a valid driver’s license are essential.

Qualifications

• 1 year experience as an LPN in home care, med-surg, or ED.
• Current LPN license.

Responsibilities

• Plan and coordinate total patient care with RN/physician and follow care plan.

Compensation

Pay per visit ($38 - $40 per routine visit), Mileage reimbursed at $0.67/mile, Payroll processed bi-weekly (26 pay periods).

Shift Type

PRN (as needed), Minimum availability for 6 visits per week, Flexibility, In the field.

Summary

We are looking for a compassionate Home Health Licensed Practical Nurse (LPN) to work one-on-one with our patients in their home environment to give proper education and skilled nursing to allow our patients to become more independent. The Home Health Licensed Practical Nurse (LPN) for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.

Responsibilities

• Participate in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care, and follow the nursing care plan for each patient.

• Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change.

• Prepare clinical/progress notes and summaries.

• Assist the patient with the activities of daily living while teaching appropriate self-care techniques.

• Follow universal precautions and all emergency procedures whenever giving any aspect of patient care.

Qualifications

• 1 year experience as a Licensed Practical Nurse (LPN) in a home care, med surg, or ED setting.

• Current state license as a Licensed Practical Nurse (LPN).

• Proof of current CPR certification and valid Driver’s license.

• Ability to work and flexible schedule.

• Excellent interpersonal skills.
