Job summary

Apollo Medical Home Health invites compassionate LPNs to provide skilled nursing to patients in their homes, delivering education and care with a patient-centered approach. You will work one-on-one with patients, surrounded by a supportive clinical team and a culture focused on high-quality outcomes.

This PRN role requires a minimum of 6 visits per week and offers mileage reimbursement, tablet support, and bi-weekly payroll, with pay per routine visit between $38 and $40 and a strong emphasis on

Qualifications

• 1 year experience as a Licensed Practical Nurse (LPN) in a home care, med surg, or ED setting.
• Current state license as a Licensed Practical Nurse (LPN).
• Proof of current CPR certification and valid Driver’s license.
• Ability to work a flexible schedule.
• Excellent interpersonal skills.

Responsibilities

• Participate in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care, and follow the nursing care plan for each patient.
• Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change.
• Prepare clinical/progress notes and summaries.
• Assist the patient with the activities of daily living while teaching appropriate self-care techniques.
• Follow universal precautions and all emergency procedures whenever giving any aspect of patient care.

Compensation:

Pay per visit ($38 - $40 per routine visit), Mileage reimbursed at $0.67/mile, Payroll processed bi-weekly (26 pay periods).

Summary:

We are looking for a compassionate Home Health Licensed Practical Nurse (LPN) to work one-on-one with our patients in their home environment to give proper education and skilled nursing to allow our patients to become more independent. The Home Health Licensed Practical Nurse (LPN) for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.
