Job summary

Evolve Therapy Services, LLC seeks a Physical Therapist for an in-house Skilled Nursing Facility in Johnstown, PA. This PRN position delivers high-quality resident care in collaboration with an interdisciplinary team.

Responsibilities include providing PT services, documenting per procedures, evaluating residents, developing treatment plans, and communicating progress to residents and families. NetHealth EMR proficiency is preferred.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Assume responsibility for ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice.
• Adhere to procedures regarding documentation and billing of physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of therapy programs.
• Conduct screenings of residents to determine need for intervention/treatment.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings and recommendations to residents, families, and interdisciplinary team members.
