Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapist to support an in-house Skilled Nursing Facility in Hastings, PA. The PRN position requires delivering high-quality PT services, evaluating residents, and coordinating with interdisciplinary teams to optimize resident outcomes.

The successful candidate will hold a BS in Physical Therapy, maintain an active state license, and participate in ongoing continuing education.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of resident therapy programs.
• Screen residents regularly to determine need for intervention/treatment and for referrals to therapy.
• Develop treatment plans with short and long-term goals and communicate findings to residents and families.
