Evolve-Therapy-Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility. The role involves planning, directing, and delivering physical therapy services to residents in accordance with federal, state, and local standards.

Responsibilities include conducting screenings, developing treatment plans, documenting per CMS expectations using NetHealth EMR, and collaborating with families and the care team to optimize resident outcomes.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents according to established standards.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate residents to inform treatment planning and implementation.
• Develop treatment plans with short and long-term goals.
• Communicate findings and plans to residents, families, and inter-disciplinary team.
