Job summary

Evolve Therapy Services, LLC is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Erie, PA. Full Time, Part Time, or PRN positions are available.

You will provide comprehensive PT services, perform evaluations, and develop treatment plans within CMS guidelines. Responsibilities include patient screening, documentation, and collaboration with a multidisciplinary team to ensure high-quality resident care.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and departmental procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of resident therapy programs.
• Screen residents regularly to determine need for intervention or treatment.
• Screen residents referred to physical therapy to determine need for intervention or treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals and methods to achieve them.
• Communicate evaluation findings and treatment plans to residents, families, and the interdisciplinary team.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR.
• Perform duties to discharge responsibilities in the best interests of the organization.
