Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Bradford, PA. This PRN position supports resident care and ensures therapy services align with all regulatory standards.

You will assess patients, develop care plans, and collaborate with an interdisciplinary team. A current PT license and ongoing education are required to maintain high-quality, compliant care.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Document and bill therapy services in accordance with CMS and company procedures.
• Evaluate residents to inform treatment planning and progress.
• Develop treatment plans with short- and long-term goals to meet identified objectives.
• Communicate evaluation findings and recommendations to residents, families, and the care team.
