Cherish Hospice Corp in Springfield, Ohio is seeking a PRN Social Worker to provide psychosocial support to patients and families during transitions. The role involves home visits, care coordination with the interdisciplinary team, and flexible scheduling.

Ideal candidates hold an MSW and Ohio license, with hospice experience, strong communication, crisis intervention skills, cultural sensitivity, and a compassionate, patient- and family-centered approach.

Competenze

• MSW degree and OH license required.
• Minimum 1 year hospice/palliative care experience preferred.

Mansioni

• Conduct psychosocial assessments within 5 days of admission.
• Participate in IDG meetings with the interdisciplinary team.
• Observe, record, and report changes in emotional and social well-being.
• Assess coping with the dying process and provide support.
• Evaluate the effectiveness of social work interventions and adjust.
• Participate in discharge planning as needed.
