Job summary

Serenity Hospice is seeking a licensed Social Worker to deliver psychosocial, emotional, spiritual, and bereavement services to hospice patients and families. You will participate in initial assessments, ongoing counseling, and education, supporting the interdisciplinary group in a home-care setting.

Qualifications include an Ohio license and an MSW from a CSWE-accredited program, with prior healthcare exposure preferred. Schedule is day shift, Monday through Friday, on the road.

Qualifications

• Licensed Social Worker in Ohio (required).
• MSW from CSWE-accredited program with at least 1 year of health-care experience preferred.

Responsibilities

• Perform initial psychosocial, emotional, spiritual, and bereavement assessments and aid in goal-directed IDG care planning.
• Conduct ongoing reassessments of patient and family needs and provide counseling as required.
