Job summary

Optimal Care is seeking a Hospice Medical Social Worker to provide compassionate, clinical psychosocial care for patients and families navigating end-of-life illness. You will complete psychosocial assessments, offer counseling, and support advance care planning within a dedicated interdisciplinary team in Northwest Ohio.

This role requires an MSW (CSWE-accredited) and active licensure, with at least 1 year of licensed MSW experience, and willingness to travel to homes, facilities, or community

Qualifications

• MSW or doctoral degree from CSWE-accredited school of social work.
• Current State License as MSW/LMSW/LCSW.
• Minimum 1 year of experience as licensed MSW.

Responsibilities

• Provide psychosocial assessments and counseling.
• 1: Crisis intervention and support.
• 2: Advance care planning conversations with families.
