Job summary

Buckeye Hospice and Palliative Care Services seeks a Medical Social Worker in Ohio to provide psychosocial support to patients and families under the Plan of Care.

Responsibilities include psychosocial assessments within 5 days of admission, participation in IDG meetings, documentation, and discharge planning. Must be licensed in Ohio; MSW or BSW with supervision and hospice experience preferred. Driver with reliable auto required.

Qualifications

• Must be licensed to practice social work in Ohio.
• MSW degree required or BSW with 1 year of healthcare social work experience.
• BSW requires MSW supervision.
• 1 year social work experience for BSW; 1 year MSW experience preferred.
• Hospice experience preferred.
• Must be a licensed driver with an insured automobile.
• Must be able to communicate well, both verbally and in writing.

Responsibilities

• Conduct psychosocial assessment within 5 days of admission to identify problems, interventions and goals for the patient’s plan of care.
• Participate in IDG meetings and the development of the patient’s plan of care.
• Observe, record and report changes in emotional and social factors affected by illness and care needs.
• Maintain and submit written clinical records including initial evaluation, care plan and daily notes.
• Evaluate patient and family response to social work intervention.
• Confirm weekly scheduling of visits with the patient’s Case Managers to coordinate other staff visits.
• Participate in IDG and QAPI meetings and activities.
• If the patient/family declines Social Work services, continue to offer support to the IDG.
• Participate in discharge planning, as needed.
• Assess the patient/family’s ability to cope with dying.
• Act as consultant to the health team on social factors related to health problems.
• Help patients utilize family and community resources.
